Position Description

ChristianaCare is currently seeking a part-time, Registered Nurse for our Diabetes Educator position to support our Springside and Smyrna practices. In this role, you will be responsible for managing diabetes patients according to Diabetes Disease Management Protocols, provide diabetes education and coordination of services, and work in collaboration with all physician, nurse practitioner, nursing, and ancillary staff of the Endocrinology specialty practice.  Coordinate and provide diabetes education and care of gestational diabetic patients for the practice. 

Work Schedule

  • Day shift, Mon-Fri
  • 32 hours per week

PRINCIPAL DUTIES AND RESPONSIBILITIES:   

  • Demonstrates knowledge and skills necessary to provide care appropriate to adult and geriatric diabetes patients including the ability to obtain and interpret information to identify patient needs and provide the care needed.
  • Executes Diabetes Disease Management Protocols for the Endocrinology Practice.
  • Monitors patient glycemic control and adjust treatments including diabetes meal planning, exercise and medications according to Diabetes Disease Management Protocol between scheduled office visits.
  • Acts as a diabetes expert resource for patients, families, staff, residents and attending physicians
  • Coordinates the Gestational Diabetes component of the Christiana Care Diabetes Management Program including case management, education, diabetes self-management training, and data base management.
  • Develops and individualizes plan of care based on patient assessment.
  • Identifies potential problems for which the patient is at risk, notifies physician and discusses plan of care according to patient’s needs.
  • Collaborates regularly with Endocrinology physicians and nurse practitioner staff regarding diabetes patient care issues including: Interpretation of CGM and self-monitoring of blood glucose data, pump download data, patient’s understanding of their disease state and self-management abilities. May assist with injections as needed.
  • Assists staff with prescription line and coordinates pharmacy refills.
  • Performs assigned work safely, adhering to established departmental safety rules and practices.  Reports any unsafe activities, conditions, hazards, or safety violations to the supervisor in a timely manner.
  • Performs other related duties as required.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Registered Nurse with Bachelor’s degree required.
  • Three to five years of clinical experience in diabetes care and/or strong interest in diabetes management.
  • Certified Diabetes Care and Education specialist or working toward certification

SPECIAL REQUIREMENTS:

  • Current RN license from the State of Delaware
  • BCLS Certification
  • Certification as a Diabetes Care and Education Specialist through the National Certification Board for Diabetes Care and Education or a commitment to obtain Certification within 3 years of starting position.
